Academic Resume Requirements

Formatting note: There is no required format for submission. Most submissions will be between 1-2 pages. (Examples: High School Academic Resume | Homeschool Transcript)

  • Student Information:
    • Name
    • Date of birth
    • Address
    • Other identifying information
  • Homeschool Information:
    • School name
    • School location
  • Course Information:
    • Names of all courses taken
    • Dates of all courses taken
    • Description of how classes fulfill CADRS
    • Number of credits earned (1 credit = 1 year)
    • Grades earned in all courses taken
    • Books or curriculum used (This is for any classes NOT taken through a college or accredited high school/program.)
  • Where the student will be earning their diploma from
  • Name, date, and signature of person responsible for the curriculum
